Useful Tips


1. Secure and submit 4 copies (3 copies if coming from hospital) of fully accomplished Death Certificates to MMCI within 24 hours.

2. Be informed: Autopsy is necessary for Medico Legal cases such as untimely death, accident, DOA, 65 years old and below, or as requested by the family.

3. Provide MMCI the clothes for your deceased loved one, the false teeth (if applicable), and a recent picture to guide the embalmer as soon as possible.

4. Inform the Service Specialist if you wish to retain any or certain facial mark(s) or hair (i.e. mustache) of your deceased loved one.

5. Confirm intended place of interment/cremation for processing of burial permit and scheduling of funeral services.  Then, arrange time and date of burial with your memorial park agent and MMCI Office.

6. Verify that the Death Certificate you have bears the seal/stamp and civil registry number of the municipality where the your deceased loved one passed away.  Only the Certified True Copy of the Death Certificate is needed for all official transactions.

7. Request assistance from MMCI office to look for priest, police escort, family car, service shuttle, memorial lot, columbarium, and facilitate cremation.  If available, these outside services entail extra charges.

8. Ask any of the Service Specialist should you need assistance with publishing obituary notices in the Philippine Star, Philippine Daily Inquirer, and/or Manila Bulletin.

 

For Memorial Plan Holders:

- All requests should be addressed to your plan’s servicing department or service representative.

- Please request acknowledgment receipts from your plan’s office.   The official receipts MMCI issues are only for clients who avail of other services like upgrade, chapel use extension, conveyance, multi-viewing, air-condition usage, etc.
